Which option best meets your needs? (required)
If you have difficulty accessing or understanding the survey on your own, we can offer assistance to help you complete it.
If you need more time or would like to save your progress and come back later, we can send you a unique link to a version of the survey that allows you to complete it over multiple sessions.
If you find it easier to complete the survey by speaking with someone, we can schedule a phone or video interview where the survey will be completed with you verbally
If you would like a paper copy of the survey mailed to you, along with prepaid envelopes for return, let us know how many copies you need
