External Disclosure Request - Government Entities

Government institutions, agencies or local authorities can make an access to information request under Sections 28 of the Act.

 Authorized government officials are required to submit this online form ‘Request for Disclosure of Personal Information to Government Entities’ with any and all mandatory information to the City Clerk’s Office for approval prior to the release of the requested information by the record custodian(s).

 Similarly, Saskatoon Police Services Officers are required to submit this online form Saskatoon Police Service Law Enforcement Disclosure Request SP2-132 to the City Clerk’s Office for approval when requesting personal information, records or access to CCTV surveillance footage for their investigation.
